How to read this record
Every claim is tagged
Most family trees flatten everything into the same confident typeface. This one does not.
The three research tags below are the whole point of the exercise: they say what a document
proves, what the weight of evidence suggests, and what nobody has found yet. A fourth marks
what the family stated directly, where no document has been retrieved at all — it is kept
separate on purpose and is never quietly promoted to confirmed. A fifth, added on
13 September 2026, marks biological evidence: a DNA result is not a document, and it is not
testimony either. Two further labels mark where this page withholds information
rather than lacking it.
ConfirmedAttested by a document that was actually retrieved and read — a census sheet, a
government index, a grave marker, a family-written obituary.
ProbableStrongly supported but not nailed down: a user-contributed tree, a masked index field,
or a chain of consistent circumstances. Believable, not proven.
Not foundSearched for and not located. This is a statement about the search, not about the
person — the record may well exist somewhere unsearched.
Family testimonySupplied by the family and recorded on their word — no document has been retrieved for
it. Note the diamond rather than the dot: this is provenance, not proof, and it stands
apart from the three record tags above S24.

Daily Review of 11 June 1946, page 1, names him as William B. O’Farrell of 1525 170th Avenue, Ashland,
“husband of Mrs. Helen O’Farrell”, the same address Helen gave in 1942

Driving home from his job at the Pacific Ice Company at about 6:40 p.m., his car crossed the centre line and hit a 10-ton truck loaded with steel beams head-on, on San Leandro Boulevard at 92nd Avenue, Oakland. The trailer overturned and spilled the steel into the street; he was dead on arrival at Highland Hospital
S1178S1180. The family remembered it as a steel-laden truck that dropped its load onto his car (Patricia (Proeger) Collins, 1 October 2026). The papers agree in substance; the difference is that his car struck the truck and the trailer then overturned. He was 46. ⚠️

🆕 A studio portrait of two couples from Patricia Collins’s collection shows Helen with her sister Mildred and Mildred’s husband Frank Da Silva Jr; Patti names the fourth person as “her husband Victor”. ⚠️ The date and the names conflict, and this record does not settle it: the dress suggests about 1915–1920, Frank Jr died in December 1924, and Helen was still married to Farrell until his death in June 1946. So the man with Helen may be her first husband, William Farrell, named by the family after the husband they knew. In Photos below.
